🔬 Key Responsibilities

✅ Provide comprehensive primary care, including patient assessments, screenings, diagnosis, treatment, and preventive services.

✅ Conduct physical examinations and obtain detailed medical histories.

✅ Develop individualized treatment plans and prescribe appropriate medications.

✅ Monitor patient progress and coordinate care with the clinical team.

✅ Educate patients on disease prevention, medication adherence, and healthy lifestyle choices.

✅ Collaborate with physicians, behavioral health providers, and other healthcare professionals.

✅ Maintain accurate and timely documentation in the electronic health record (EHR).

✅ Coordinate specialist referrals and consultations.

✅ Participate in team meetings, clinician cross-coverage, and the after-hours on-call rotation.

📋 Required Qualifications

🎓 master's degree or doctoral degree from an accredited Nurse Practitioner program.

📜 Current, unrestricted California Nurse Practitioner license.

🏆 Board certification appropriate to the Family Nurse Practitioner role.

💊 Active, unrestricted DEA license.

❤️ Current BLS certification.

🩺 Minimum 2 years of primary care clinical experience required.

⭐ Preferred: Experience in a community health center, FQHC, or healthcare setting serving underserved populations.
