We are seeking a compassionate and qualified Family Nurse Practitioner (FNP) to join a primary care team providing comprehensive healthcare to patients of all ages, from newborns to older adults.
This opportunity is ideal for an FNP who enjoys building long-term patient relationships, managing acute and chronic medical conditions, and promoting preventive health and wellness.
Job Responsibilities
Perform comprehensive physical examinations and obtain patient medical histories.
Diagnose and manage acute illnesses, minor injuries, and chronic conditions, including diabetes and hypertension.
Develop individualized treatment plans and prescribe medications within the scope of practice.
Provide routine wellness visits, immunizations, screenings, and preventive healthcare services.
Educate patients and families on disease prevention, medication management, nutrition, exercise, and healthy lifestyle choices.
Monitor treatment progress and adjust care plans as appropriate.
Perform minor office procedures within the scope of practice.
Maintain accurate medical records and collaborate with physicians and other healthcare professionals to coordinate patient care.
Qualifications
Family Nurse Practitioner certification and applicable state licensure.
Strong clinical assessment, diagnostic, and decision-making skills.
Excellent communication and interpersonal abilities.
Commitment to patient-centered care and preventive medicine.
Ability to care for patients across the lifespan.
