We seek nurse practitioners to fulfill contract opportunities with Tacoma-Pierce County. These are IMMEDIATE NEEDS starting as soon as possible! The practitioner will work with a multidisciplinary team in a providing quality healthcare throughout the community. Please review details below:
Modality: Community Health Nurse Practitioner
Openings: (3) Full Time Nurse Practitioners
Contract Length: 3 – 6 months (option to extend)
Start Date: ASAP
Job Responsibilities
- Provide respectful, culturally appropriate outreach and patient care in the field (e.g., at encampments, streetside for people living in cars).
- Services include a broad range of primary care and urgent care issues, such as wound care, chronic disease management, mental health care, and substance use care.
- Coordinate with local healthcare organizations, including opioid treatment programs, to help people transition into traditional facility-based care.
- Work with certified peer navigator and RN to coordinate patient care.
- Build collaborations with community partners and organizations involved in homeless services.
- Develop and review Street Medicine policies and procedures, including standing orders if needed.
Qualifications
• Current DEA
• Experience in primary, urgent and wound care a PLUS
Working Hours
Hours per Week: 40
