Nurse Practitioner – Primary Care – San Jose, CA

Compensation: $74.88–$86.81/hour

We are seeking an experienced Nurse Practitioner to join a multidisciplinary primary care team in San Jose, CA. This position focuses on providing comprehensive outpatient care and may also include women’s health, family planning, and other community-based services.

Responsibilities

• Evaluate, diagnose, and treat primary care patients

• Perform comprehensive physical assessments and examinations

• Develop appropriate treatment and care plans

• Prescribe medications within scope of practice

• Order and interpret laboratory testing

• Provide preventive care and patient education

• Coordinate referrals to physicians, specialists, and outside services as needed

• Maintain accurate and timely electronic medical records

• Collaborate with physicians, nurses, medical assistants, and other members of the care team

Qualifications

• Current California Nurse Practitioner license

• National NP certification through AANP or another recognized certifying body

• Completion of an accredited Nurse Practitioner program

• Minimum of 2 years of clinical experience

• Strong assessment, diagnostic, and patient-care skills

• Ability to work independently and collaboratively within a multidisciplinary team
