Job description
Client is seeking a Family Nurse Practitioner (FNP) to join its clinical team and provide comprehensive family medicine services to patients across the lifespan in a rural South Dakota community.
This role offers broad clinical exposure, combining preventive care, acute illness management, chronic disease management, wellness visits, and ongoing patient follow-up. The FNP will work collaboratively with physicians, APPs, nurses, and ancillary healthcare professionals in a rural medical center environment connecting outpatient family medicine with hospital services.

Job Responsibilities
Conduct comprehensive new-patient, wellness, annual, and follow-up visits
Obtain medical histories and perform physical examinations
Diagnose and treat acute illnesses and common injuries
Manage chronic conditions including diabetes, hypertension, thyroid disorders, and other ongoing conditions
Develop individualized treatment plans and monitor patient progress
Order and interpret laboratory, imaging, and diagnostic studies
Prescribe medications within applicable authority
Provide preventive screenings, counseling, and patient education
Coordinate referrals and communicate with specialists
Support continuity of care following emergency visits, hospitalizations, and discharge
Maintain accurate clinical documentation
Recognize changes in patient condition and arrange appropriate consultation or escalation
Participate in quality improvement and patient safety initiatives
Collaborate closely with physicians, APPs, nurses, and support staff
Hospital & Acute Care Involvement:
The broader healthcare environment connects outpatient family medicine with hospital services. Depending on the final role structure, the FNP may participate in hospital-based care, acute evaluations, emergency coverage, or call responsibilities. Specific expectations will be based on education, certification, demonstrated competencies, and approved facility privileges.
Qualifications
MSN, DNP, or qualifying graduate degree from an accredited Nurse Practitioner program
Current national FNP certification
Current South Dakota RN/APRN licensure or eligibility to obtain
DEA registration or eligibility to obtain
Strong assessment, diagnostic reasoning, and treatment-planning skills
Ability to provide preventive, acute, and chronic care across the lifespan
Strong communication and documentation skills
Sound clinical judgment and ability to recognize when consultation is appropriate
Preferred Experience:
Family medicine or rural healthcare experience
Community-based primary care
Critical access hospital experience
Urgent care, emergency, or inpatient experience
Management of patients with multiple chronic conditions
Experience working across outpatient and hospital settings
