Job description
We are seeking a qualified and compassionate Family Nurse Practitioner (FNP) to provide comprehensive, family-focused primary care to patients across the lifespan, from newborns through older adults.
Job Responsibilities
Diagnose and manage acute conditions, infections, minor injuries, and sprains.
Diagnose and manage chronic conditions, including diabetes and hypertension.
Develop individualized treatment plans and prescribe medications within scope of practice.
Provide preventive care, routine check-ups, immunizations, wellness screenings, and health assessments.
Counsel patients on nutrition, exercise, wellness, and disease prevention.
Perform minor office procedures as appropriate.
Monitor treatment progress and adjust care plans when needed.
Educate patients and families regarding medications, treatment options, and healthy lifestyle choices.
Maintain accurate and complete medical records.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care
Qualifications
Family Nurse Practitioner (FNP) with strong clinical assessment and decision-making skills.
Excellent communication and interpersonal abilities.
