Compensation: $110,000–$120,000 Base Salary + Bonus Opportunity

Position Responsibilities

• Provide comprehensive primary care services to geriatric patients in long-term care, skilled nursing, and assisted living communities.
• Perform patient assessments, diagnose acute and chronic medical conditions, and develop individualized treatment plans.
• Manage chronic diseases while promoting preventative care and wellness.
• Collaborate closely with physicians, nursing staff, therapists, and interdisciplinary care teams.
• Maintain timely, accurate, and compliant medical documentation.
• Communicate effectively with patients, families, and facility staff to ensure continuity of care.
• Participate in care planning and quality improvement initiatives designed to enhance patient outcomes.

Qualifications

Required

• Licensed Nurse Practitioner or Physician Assistant (or eligible for state licensure)
• Excellent communication and interpersonal skills
• Compassionate, patient-centered approach to care
• Ability to work both independently and collaboratively within a multidisciplinary team

Preferred

• Experience in Geriatrics, Primary Care, Internal Medicine, Family Medicine, Long-Term Care, Skilled Nursing, or Assisted Living
• Behavioral Health experience is a plus
