Primary Care Nurse Practitioner

Salary Range: $130,000-$175,000

A well-established multispecialty practice is seeking an experienced Nurse Practitioner to deliver high-quality outpatient primary care. This role offers the chance to build lasting patient relationships in a collaborative, physician-led environment. You'll be supported by an integrated network of primary care physicians, cardiologists, endocrinologists, and other specialists.

You'll step into an established patient population rather than building a panel from scratch. The practice also runs a dedicated same-day service for acute needs, and its internal specialty network keeps referrals efficient and communication close. That means you can focus on thorough, continuous care.

Responsibilities

• Conduct comprehensive patient evaluations and physical examinations
• Manage acute visits for a broad range of primary care concerns
• Provide ongoing chronic disease management, including conditions such as hypertension, diabetes, and hyperlipidemia
• Deliver preventive care, including screenings, wellness visits, and patient education
• Order and interpret diagnostic tests, labs, and imaging
• Prescribe and manage medications
• Coordinate specialist referrals and collaborate closely with physicians across the network
• Document care accurately and in a timely manner in the EHR

Qualifications:

• Board certification: FNP or AGNP in New York State
• Active DEA registration, or eligibility to obtain one
• Outpatient primary care experience preferred
• Current BLS certification
• Strong clinical judgment, communication skills, and a patient-centered approach
