We are seeking a Family Nurse Practitioner (FNP) to provide comprehensive primary care to patients across the lifespan. This role is ideal for an NP who values preventive care, chronic disease management, patient education, and individualized treatment plans.

What You’ll Do

• Provide comprehensive primary care across the lifespan
• Evaluate and manage acute and chronic conditions
• Perform health assessments and order/review diagnostic testing
• Provide preventive care and health-promotion services
• Develop personalized treatment and wellness plans
• Counsel patients on nutrition, lifestyle, and prevention
• Build long-term relationships with patients and families
• Coordinate care with specialists and other clinicians
• Maintain accurate clinical documentation and EHR records

What We’re Looking For

• Current, unrestricted NP license and certification
• FNP preferred
• Eligible to practice as an NP in Ohio
• Master’s or Doctoral degree in Nursing from an accredited program
• Primary care, family medicine, or outpatient experience
• Experience managing chronic conditions and preventive care
• Strong clinical assessment and diagnostic reasoning skills
• Excellent communication and patient relationship skills
• Interest in integrative, lifestyle, or whole-person medicine is highly valued
