Family Nurse Practitioner – Rural Family Medicine | Faulkton, SD | Full-Time
Location: Faulkton, South Dakota
Setting: Rural Medical Center / Family Medicine
Schedule: Full-time, onsite

About the Opportunity
Join a collaborative rural healthcare team providing comprehensive family medicine to patients across the lifespan. This role offers broad clinical variety, long-term patient relationships, and the opportunity to become a trusted healthcare resource within a close-knit community.
Job Responsibilities
Provide comprehensive primary care, wellness, preventive, acute, and chronic disease management.
Evaluate patients, perform physical examinations, establish diagnoses, and develop treatment plans.
Manage conditions including diabetes, hypertension, thyroid disorders, and other chronic illnesses.
Order and interpret labs, imaging, and diagnostic studies.
Prescribe and manage medications within applicable authority.
Provide patient education, preventive counseling, and follow-up care.
Coordinate referrals and collaborate with physicians, APPs, nurses, and ancillary staff.
Support continuity of care following emergency visits, hospitalizations, and discharges.
Participate in quality improvement and patient-safety activities.
Depending on final assignment and privileges, hospital, emergency, or call responsibilities may be
Qualifications
MSN, DNP, or qualifying graduate degree from an accredited NP program.
Current national FNP certification.
Current South Dakota RN/APRN license or eligibility to obtain.
DEA registration or ability to obtain when required.
Strong assessment, diagnostic reasoning, treatment-planning, communication, and documentation skills.
Ability to provide preventive, acute, and chronic care across the lifespan.
Preferred
Family medicine, rural health, or community primary care experience.
Critical access hospital experience.
Urgent care, emergency, or inpatient experience.
Experience managing medically complex patients.
Ability to adapt to varied clinical needs in a rural setting.
