Job Responsibilities
As a Family Nurse Practitioner, you will help patients navigate both everyday healthcare needs and more complex medical concerns.
Your work will combine comprehensive assessment, individualized treatment planning, patient education, and ongoing follow-up. By developing long-term relationships with patients, you will gain an understanding of their medical histories, goals, family circumstances, and barriers to care.
Whether evaluating an acute illness, adjusting a chronic disease treatment plan, or helping a patient take preventive steps, you will play an important role in maintaining access to quality healthcare close to home.
Primary Care Responsibilities:
Provide comprehensive evaluations for patients across the lifespan, including new-patient visits, annual examinations, wellness visits, and follow-up care.
Obtain detailed medical histories and perform physical examinations.
Develop differential diagnoses and establish appropriate treatment plans.
Evaluate and treat acute illnesses and common injuries within the scope of practice.
Manage chronic conditions such as diabetes, hypertension, thyroid disorders, and other ongoing healthcare needs.
Order and interpret appropriate laboratory studies, imaging, and diagnostic testing.
Prescribe and manage medications within applicable licensure and prescribing authority.
Monitor treatment response and adjust care plans based on clinical findings and patient progress.
Provide age-appropriate preventive screenings, counseling, and health education.
Educate patients regarding diagnoses, treatment options, medications, and follow-up requirements.
Coordinate referrals and communicate with specialists when additional evaluation or treatment is needed.
Care Coordination & Team Collaboration:
Work closely with physicians, advanced practice providers, nurses, and support staff to provide coordinated patient care.
Recognize changes in patient condition and arrange timely consultation, escalation, or transfer when appropriate.
Support continuity of care following emergency visits, hospital admissions, and discharge.
Communicate effectively with patients, families, caregivers, and members of the healthcare team.
Maintain accurate and timely documentation of assessments, treatment decisions, and follow-up plans.
Participate in clinical discussions, quality-improvement initiatives, and patient-safety activities.
Contribute to a professional, accountable, and respectful team environment.
Hospital & Acute Care Involvement:
Client's broader healthcare environment provides a connection between outpatient family medicine and hospital services.
Depending on the final assignment, the FNP may participate in:
Hospital-based care
Acute evaluations
Emergency coverage
On-call responsibilities
Specific responsibilities will be aligned with the FNP's education, certification, demonstrated competencies, and approved facility privileges.
Schedule, coverage expectations, and available clinical support will be discussed during the interview process.
Qualifications
MSN, DNP, or another qualifying graduate degree from an accredited Nurse Practitioner program.
Current national certification as a Family Nurse Practitioner.
Current South Dakota RN/APRN licensure, or eligibility to obtain the required credentials before starting.
DEA registration or the ability to obtain it before performing duties requiring DEA.
Strong clinical assessment, diagnostic reasoning, and treatment-planning skills.
Ability to provide preventive, acute, and chronic care across the family medicine population.
Strong communication and clinical documentation skills.
Ability to exercise sound clinical judgment, recognize limitations, and seek consultation appropriately.
Commitment to compassionate care and the healthcare needs of a rural community.
Preferred Background:
Experience in family medicine, rural health, or community-based primary care.
Experience in a critical access hospital or healthcare setting connecting outpatient and hospital services.
Urgent care, emergency, or inpatient experience relevant to the final assignment.
Experience managing patients with multiple chronic conditions.
Comfort adapting to varied clinical needs and collaborating across disciplines.
Demonstrated ability to establish patient trust and maintain continuity of care.
