About the Opportunity
We are seeking a compassionate and qualified Family Nurse Practitioner (FNP) to join our growing primary care team. This is an excellent opportunity for an advanced practice provider who is passionate about delivering comprehensive, patient-centered healthcare to individuals and families across the lifespan.
The ideal candidate will provide preventive, acute, and chronic care management while building meaningful relationships with patients and their families. You will work in a collaborative clinical environment focused on wellness, education, and long-term health outcomes.
Job Responsibilities
Conduct comprehensive patient assessments, physical examinations, and health histories
Diagnose and treat acute illnesses and minor injuries
Manage chronic conditions such as diabetes, hypertension, and other ongoing health concerns
Develop individualized treatment plans based on patient needs
Prescribe medications and therapies within scope of practice
Provide preventive care, wellness visits, and health screenings
Administer and recommend immunizations
Educate patients and families regarding disease prevention, treatment options, medications, nutrition, and healthy lifestyle choices
Monitor treatment effectiveness and adjust care plans as appropriate
Perform minor office procedures within scope of practice
Maintain accurate and timely medical documentation
Collaborate with physicians and interdisciplinary healthcare professionals to ensure coordinated patient care
Qualifications
Board-Certified Family Nurse Practitioner (FNP)
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Patient-focused and compassionate approach to care
Ability to manage diverse patient populations across all age groups
Commitment to preventive medicine, wellness, and long-term health outcomes
Ability to thrive in a collaborative outpatient environment
