Family Nurse Practitioner – Integrative Primary Care | Membership Medicine
An established membership-based primary care practice in the Cincinnati metropolitan area is seeking a compassionate and clinically skilled Family Nurse Practitioner (FNP) to join its growing team.
This is a full-time, on-site opportunity for a provider who enjoys comprehensive primary care and has an interest in whole-person, integrative, and lifestyle-focused medicine.
The practice emphasizes personalized care, prevention, patient education, longer-term relationships, and individualized treatment planning. Providers have the opportunity to spend meaningful time with patients while addressing both immediate healthcare needs and broader factors that may contribute to long-term health.
Job Responsibilities
Provide primary care to patients across the lifespan.
Perform comprehensive health assessments, physical examinations, and clinical evaluations.
Diagnose and manage acute and chronic conditions.
Develop individualized, evidence-informed treatment plans.
Order, review, and interpret appropriate laboratory and diagnostic studies.
Provide preventive care, screenings, and health-promotion services.
Monitor patient progress and modify treatment plans as appropriate.
Whole-Person & Integrative Care
Incorporate a whole-person perspective into patient evaluation and care planning.
Consider underlying contributors to health concerns as part of comprehensive assessment.
Incorporate appropriate lifestyle-based interventions into patient care.
Counsel patients regarding nutrition, exercise, lifestyle, prevention, and long-term wellness.
Develop care plans aligned with individual patient goals.
Patient Relationships & Education
Establish meaningful, long-term relationships with patients and families.
Communicate diagnoses, treatment options, and recommendations clearly and compassionately.
Educate patients regarding preventive care and healthy lifestyle strategies.
Provide ongoing follow-up and support within a membership-based care model.
Encourage patients to actively participate in their healthcare decisions.
Care Coordination & Collaboration
Coordinate care with specialists and other healthcare professionals when appropriate.
Participate in interdisciplinary clinical discussions.
Collaborate with the care team on clinical protocols and patient-care processes.
Communicate effectively regarding patient needs, treatment plans, and follow-up.
Contribute to ongoing practice development.
Documentation & Clinical Operations
Maintain accurate and timely electronic health records.
Document encounters, assessments, diagnoses, treatment plans, and follow-up.
Follow established clinical documentation and compliance standards.
Participate in quality improvement and development of clinical protocols.
Qualifications
Current, unrestricted Nurse Practitioner license and certification.
Family Nurse Practitioner (FNP) certification preferred or equivalent NP preparation.
Eligibility to practice as an NP in Ohio.
Master's or Doctoral degree in Nursing from an accredited program.
Demonstrated primary care experience.
Strong clinical assessment and diagnostic reasoning skills.
Ability to develop individualized, evidence-informed treatment plans.
Excellent communication and interpersonal skills.
Ability to establish strong, long-term patient relationships.
Proficiency with electronic health records and clinical documentation.
Ability to collaborate effectively with an interdisciplinary healthcare team.
Preferred
Prior outpatient, family medicine, or primary care experience.
Experience providing care across the lifespan.
Experience with chronic disease management and preventive care.
Interest or experience in integrative, functional, or whole-person medicine.
Experience with lifestyle-focused patient counseling.
Interest in nutrition, wellness, prevention, and long-term health optimization.
Comfort working within a membership-based/direct-pay care model.
