Family Nurse Practitioner (FNP)

Position Overview

Key Responsibilities

• Provide comprehensive preventive and therapeutic patient care across the lifespan.
• Assess, diagnose, treat, and manage acute and chronic health conditions.
• Maintain accurate and timely patient documentation using established clinical standards and SOAP format.
• Review and interpret laboratory results, diagnostic imaging, specialist reports, and referral documentation.
• Develop individualized treatment plans and coordinate appropriate follow-up care.
• Educate patients and families regarding disease prevention, wellness, medications, and treatment plans.
• Coordinate referrals and collaborate with specialists and other healthcare providers as needed.
• Follow established clinical protocols, evidence-based guidelines, and scope-of-practice regulations.
• Participate in patient outreach activities, including follow-up for chronic disease management and missed appointments.
• Support quality improvement initiatives, peer review activities, and clinical team meetings.
• Participate in rotating on-call coverage and telephone triage as required.
• Collaborate with physicians and clinical leadership regarding complex patient cases.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current Nurse Practitioner license in the State of California.
• Active BLS/CPR certification.
• Strong clinical assessment, diagnostic, and patient management skills.
• Ability to work effectively in a team-based healthcare environment.
• Excellent communication and patient education skills.
• Bilingual English/Spanish skills preferred.

Preferred Experience

• Experience providing primary care services in outpatient or community health settings.
• Knowledge of preventive medicine, chronic disease management, and population health initiatives.
• Familiarity with electronic health record (EHR) systems.

Schedule & Work Environment

• Full-time position.
• Outpatient clinical setting.
• Participation in on-call and rotating coverage schedules may be required.
