Seeking an experienced Nurse Practitioner to join a collaborative primary care team in Chowchilla, CA. This is a full-time opportunity providing comprehensive family and preventive care to a diverse community population.
Responsibilities
- Provide comprehensive primary and preventive care
- Evaluate, diagnose, and treat acute and chronic conditions
- Develop treatment plans and coordinate referrals and follow-up
- Perform appropriate office-based procedures
- Maintain accurate clinical documentation
- Collaborate with physicians and other healthcare professionals
- Participate in quality improvement and community health initiatives
Requirements
- Graduate of an accredited Nurse Practitioner program
- Active California NP license or eligibility
- 12+ months of clinical experience preferred
- Primary care/family practice experience preferred
- Strong communication and patient-care skills
