Overview

We are seeking a compassionate and patient-focused Family Nurse Practitioner (FNP) to join a growing outpatient primary care team. The provider will deliver comprehensive healthcare services, including preventive care, acute illness treatment, chronic disease management, patient education, and coordination of care for patients of all ages.

Responsibilities

• Provide comprehensive primary care services for pediatric, adult, and geriatric patients.
  
• Evaluate, diagnose, and treat acute and chronic medical conditions.
  
• Perform health assessments, physical examinations, and preventive screenings.
  
• Develop and implement individualized treatment plans.
  
• Prescribe medications and therapies in accordance with California regulations and organizational policies.
  
• Review and interpret laboratory, diagnostic, and imaging results.
  
• Coordinate referrals and collaborate with specialists and interdisciplinary healthcare teams.
  
• Educate patients and families on disease prevention, wellness, and treatment options.
  
• Maintain accurate and timely documentation within the electronic medical record (EMR).
  
• Participate in quality improvement initiatives and clinical team meetings.
  
• Provide follow-up care and ongoing management for chronic health conditions.
  
• Participate in rotating call coverage as required.
  
  Qualifications
  
• Graduate of an accredited Nurse Practitioner program.
  
• Active California Nurse Practitioner License.
  
• Current BLS/CPR certification.
  
• DEA certification or eligibility to obtain.
  
• Excellent clinical, communication, and patient education skills.
  
• Ability to work independently while collaborating effectively within a multidisciplinary team.
  
  Preferred Qualifications
  
• Experience in Family Practice, Primary Care, Community Health, or Outpatient Medicine.
  
• Bilingual English/Spanish skills preferred.
  
• Familiarity with electronic health records and population health management.
