Nurse Practitioner – Ocean Springs, MS | Epic + Mentorship | Outpatient Primary Care

Key Responsibilities:
Patient Care:
Perform comprehensive patient assessments, medical histories, and physical examinations in accordance with clinic guidelines.
Evaluate and manage patients presenting with acute and chronic conditions.
Develop appropriate care plans in collaboration with physicians and the broader clinical team.

Clinical Management:
Manage acute and chronic medical conditions using established clinical protocols and evidence-based practices.
Collaborate with physicians regarding diagnosis, treatment planning, medication management, and follow-up care.
Follow established clinical pathways for diagnostics, treatment, and patient monitoring.

Epic EMR & Documentation:
Maintain accurate, complete, and timely patient documentation within Epic EMR.
Utilize clinic-approved templates and workflows for clinical documentation.
Ensure records meet organizational standards for quality, accuracy, and compliance.

Care Coordination:
Collaborate closely with physicians, nurses, CCM staff, and other support personnel.
Help coordinate patient services and follow-up care to promote continuity and quality of care.
Communicate effectively with the care team regarding patient needs and clinical concerns.

Patient Communication & Education:
Provide patients and families with clear education regarding diagnoses, treatment plans, medications, and follow-up instructions.
Communicate with patients using clinic-approved standards and a professional, compassionate approach.
Encourage patient understanding and participation in their care plans.

Professional Development:
Participate in structured mentorship and ongoing clinical training.
Engage in chart reviews, feedback sessions, and quality-improvement activities.
Demonstrate a commitment to continuous learning and development of clinical skills.
