• Provide ongoing medical care to residents in skilled nursing and long-term care facilities.
• Manage acute and chronic medical conditions.
• Work closely with physicians and interdisciplinary care teams.

Job Responsibilities
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and therapies.
• Order and review labs, imaging, and diagnostic tests.
• Provide post-hospital and transitional care.
• Educate residents, families, and caregivers.
• Maintain accurate EMR documentation.
Qualifications
• Active, unrestricted Oregon license or ability to obtain one.
• National board certification or eligibility.
• DEA registration and prescriptive authority or ability to obtain.
• NPI and Medicare/Medicaid eligibility.
• Current BLS certification.
• U.S. work authorization.
Working Hours
