Practice Setting
Providers work in a modern outpatient clinic serving employees and their families. Unlike traditional fee-for-service practices, the focus is on preventive care, wellness, chronic disease management, and continuity of care, allowing providers more time with each patient.
Clinical Responsibilities
- Conduct annual physicals and routine wellness exams
- Diagnose and treat acute and chronic conditions
- Provide primary care services for pediatric and adult patients
- Perform women's health services, including Pap smears and preventive screenings
- Administer immunizations and vaccinations
- Order and review laboratory studies
- Coordinate care with specialists and community providers
- Educate patients on healthy lifestyle choices and disease prevention
- Document encounters utilizing Epic EMR
Schedule
- Tuesday & Wednesday: 9:00 AM - 7:00 PM
- Thursday & Friday: 6:00 AM - 4:00 PM
- 38 hours per week
- Weekday schedule only
- No weekends
- Start Date: October 2026
- 12-week contract with extension potential
Patient Volume & Support
- Average 7-12 patients per day
- RN/MA support available
- Epic EMR

Qualifications
Required
- Board-Certified Family Nurse Practitioner (FNP)
- Active Indiana Nurse Practitioner License
- Active DEA Registration
- Current BLS Certification
- Minimum 2 years of outpatient primary care experience
- Experience treating patients of all ages
