Position Overview

We are seeking an experienced Family Nurse Practitioner (FNP) to join a collaborative clinical care team in Indianapolis, Indiana.

The Nurse Practitioner will provide evidence-based primary and family healthcare, including comprehensive patient assessment, diagnosis, treatment, chronic condition management, health counseling, and care coordination.

The ideal candidate will be comfortable practicing autonomously while collaborating with a dedicated clinical care team.

Prior primary care or urgent care experience is required.

Key Responsibilities

• Provide evidence-based primary and family healthcare to patients.

• Perform comprehensive patient assessments, routine examinations, and diagnostic testing.

• Diagnose and treat illnesses, injuries, and chronic health conditions.

• Provide patient education and health counseling.

• Promote healthy lifestyle choices and preventive healthcare.

• Support health screenings, behavioral health, wellness, and chronic condition management.

• Apply quality measurement principles in chronic disease management.

• Coordinate care with physicians and other healthcare professionals.

• Manage patient appointments and follow-up care.

• Encourage patients to actively participate in their healthcare and well-being.

• Perform administrative duties including opening/closing the clinic, inventory management, insurance verification, payment collection, and maintaining a clean environment.

• Practice autonomously while collaborating effectively with the clinical care team.

Required Qualifications

• Minimum 1 year of recent Primary Care experience as a Nurse Practitioner in family medicine, primary care, or urgent care.

• Master's degree from a Family Nurse Practitioner program.

• Current national board certification.

• Current NP license in the state of employment.

• NP license must be in good standing with no current or prior disciplinary actions.

• Strong clinical judgment and ability to practice independently.

• Excellent verbal, written, and electronic communication skills.

• Strong organizational and multitasking abilities.

• Excellent initiative and problem-solving skills.

• Adaptability and flexibility in a clinical environment.

• Proficiency with electronic medical systems and healthcare technology.

• Willingness to obtain additional or multi-state licensure as needed.

• Verifiable high school diploma or GED.

Required Certifications & Documentation

• Current BLS certification is required.

• BLS certification must be issued by the American Heart Association (AHA) or American Red Cross (ARC).

• Candidates must provide documentation/screenshots of their current BLS certification and professional license.

• DEA registration may be required during onboarding.

Schedule & Training

This is a full-time position requiring approximately 38 hours per week.

Clinic Shifts May Include:

• Monday–Friday: 8:00 AM – 7:00 PM

• Monday–Friday: 8:00 AM – 4:30 PM

• Monday–Friday: 8:00 AM – 3:30 PM

• Saturday: 8:00 AM – 7:00 PM or 9:00 AM – 5:30 PM

• Sunday: 8:00 AM – 4:30 PM

• Weekend Rotation: Every other weekend

• Holiday rotations may apply.

Training:

• 40 hours per week for approximately 3 weeks
