Join a well-established Federally Qualified Health Center (FQHC) at either the Washington Heights Health Center in Manhattan or the Jamaica Center in Queens. Serve over 85,000 patients annually across the Bronx, Brooklyn, Manhattan, and Queens, providing comprehensive primary care services to underserved communities. Work within a collaborative, multidisciplinary care model alongside nursing, behavioral health, social work, nutrition, and health education teams.

Requirements

  • Family Nurse Practitioner certification
  • Active New York State NP license
  • Minimum of 1-2 years post-graduate clinical experience
  • Prior Primary Care, Ambulatory Care, Community Health, or FQHC experience preferred
  • Commitment to serving diverse and underserved populations
  • Strong team-oriented and patient-centered approach
