What You'll Do
- Provide preventive, acute, and chronic care services
- Manage a patient panel using a whole-person, value-based care approach
- Collaborate with an integrated care team to improve patient outcomes
- Support patients through education, wellness coaching, and shared decision-making
- Participate in quality improvement and population health initiatives
Qualifications
- MSN from an accredited program
- Board Certified Nurse Practitioner
- Current RN and APRN licensure in good standing
- Current DEA registration
- 3+ years of primary care experience preferred
- Team-based care experience preferred
- EMR proficiency required
