Job Responsibilities
• Conduct physical examinations, obtain medical histories, and develop individualized treatment plans.
• Prescribe and manage medications while monitoring patient outcomes.
• Educate patients on disease prevention, medication adherence, wellness, and healthy lifestyle choices.
• Collaborate with physicians, behavioral health providers, and interdisciplinary teams to coordinate patient care.
• Maintain accurate EHR documentation and coordinate specialty referrals.
• Participate in team meetings, clinician cross-coverage, and after-hours call rotation.
Qualifications
• Current, unrestricted California Nurse Practitioner license.
• Board Certified as a Nurse Practitioner.
• Active, unrestricted DEA registration.
• Current BLS certification.
• Minimum 2 years of Primary Care clinical experience required.
• FQHC, community health center, or underserved healthcare experience preferred.
• Strong communication, clinical judgment, and teamwork skills.
