Job Responsibilities
• Diagnose and manage acute and chronic medical conditions.
• Order, interpret, and document diagnostic tests and studies.
• Prescribe medications and develop individualized treatment plans.
• Provide patient education focused on prevention, wellness, and disease management.
• Collaborate with physicians, nurses, and multidisciplinary healthcare teams.
• Maintain accurate and timely electronic medical records.
• Ensure compliance with clinical, regulatory, quality, and patient-safety standards.
Qualifications
• Current DEA registration.
• Active Medicare and Medicaid provider enrollment.
• Must meet New York's 3,600-hour independent practice requirement.
• Primary Care experience required.
• Master's Degree in Nursing (MSN).
• Strong clinical judgment, communication, documentation, and organizational skills.
