Job Responsibilities
• Diagnose and manage acute and chronic medical conditions.
• Order, interpret, and document diagnostic tests and studies.
• Develop treatment plans, prescribe medications, and provide patient education.
• Maintain accurate electronic medical records and collaborate with multidisciplinary care teams.
Qualifications
• Current DEA registration.
• Active Medicare and Medicaid provider enrollment.
• Must meet New York’s 3,600-hour independent practice requirement.
• Primary Care experience required.
• MSN or DNP degree.
• Strong clinical judgment and communication skills.
