Family Nurse Practitioner

Responsibilities

• Collaborate with the Medical Director and Physician Assistant to deliver comprehensive care.
• Provide primary care services to patients ages 5 and up, with a focus on adult patients.
• Conduct women's wellness visits and coordinate care with the onsite/affiliated GYN provider as needed.
• Utilize a standard scheduling template for efficient patient management, including 20-minute follow-ups and 40-minute new patient/complex visits.
• Participate in a shared call rotation of 1:7 to ensure continuous patient care.
• Operate independently, leveraging prior experience and expertise in primary care.
• Engage with patients and families connected to OPWDD services, providing compassionate and tailored care.

Requirements

• Board Certified Family Nurse Practitioner (FNP) required.
• Active New York license or ability to obtain one.
• Prior primary care experience strongly preferred.
• Experience in OPWDD, FQHC, community health, Medicaid/Medicare, or with underserved populations preferred.
• Experience caring for patients with intellectual/developmental disabilities is highly desirable.
• Ability to manage a primarily adult patient population while caring for ages 5 and up.
• Comfortable participating in a shared call schedule.
• Strong preference for candidates requiring limited mentorship and onboarding support.
