As part of one of the largest non-profit, fully-integrated healthcare systems in Connecticut, the facility utilizes state-of-the-art clinical technology including EPIC, promoting improved patient outcomes and streamlined continuity of care.Opportunity:
Advanced Practitioners - Primary CareJob Details:
- Join an established family medicine practice caring for patients from birth through adulthood in a 100% outpatient setting
- Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA)
- Full-time position with a four-day work week option available
- Weekdays only schedule with no call required
- Serve a diverse, multicultural patient population including many Spanish-speaking families
- Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans
- Develop plans for further evaluation of health needs, including referrals to specialists and other health team members
- Provide appropriate treatment for problems within scope of practice and refer outside cases to physician as needed
- Work collaboratively with practice physicians and care team members
- Unrestricted State of Connecticut Medical License required (or eligible)
- Minimum of 1+ years of experience in a primary care or similar setting required
