Client is seeking a compassionate and experienced Family Nurse Practitioner (FNP) to join our growing primary care team in Charleston, SC.
The Family Nurse Practitioner will provide comprehensive primary care services for patients of all ages within a Direct Primary Care model. This role includes preventive care, chronic disease management, acute illness evaluation, patient education, health assessments, and care coordination.

Job Responsibilities
Primary Care Delivery:
Provide comprehensive primary care services to patients across all age groups.
Conduct health assessments, preventive care visits, wellness exams, and acute care evaluations.
Diagnose and manage chronic conditions while developing individualized treatment plans.
Evaluate patient symptoms, review medical histories, and perform clinical assessments.
Order, interpret, and review diagnostic tests and laboratory results.
Prescribe medications and provide appropriate treatment recommendations within scope of practice.
Coordinate care with specialists and other healthcare professionals as needed.
Patient Care & Education:
Build strong, long-term relationships with patients through accessible and personalized care.
Educate patients on disease prevention, treatment options, lifestyle modifications, and wellness strategies.
Provide compassionate counseling while addressing physical, emotional, and healthcare needs.
Promote patient engagement and shared decision-making.
Clinical Operations:
Maintain accurate and timely documentation in the electronic health record (EHR).
Support same-day and next-day appointment availability.
Collaborate with clinical team members to improve workflows and patient care processes.
Participate in practice initiatives focused on quality improvement and patient satisfaction.
Maintain compliance with healthcare regulations, clinical standards, and documentation requirements.
Qualifications
Master’s degree in Nursing or related advanced practice nursing education.
Current, unrestricted Family Nurse Practitioner (FNP) license in South Carolina.
National board certification as:
FNP-C (Family Nurse Practitioner – Certified), or
FNP-BC (Family Nurse Practitioner – Board Certified)
Strong clinical foundation in family medicine and primary care.
Experience providing evidence-based outpatient primary care services for patients of all ages.
Current DEA registration.
Current BLS certification.
Proficiency with electronic health record (EHR) systems.
Excellent communication, patient education, and interpersonal skills.
Ability to work independently while collaborating effectively within a small clinical team.
Preferred Qualifications:
Prior outpatient primary care or family practice experience.
Experience working in a Direct Primary Care (DPC), membership-based, or value-based healthcare model.
Familiarity with employer-sponsored healthcare programs.
Experience managing diverse patient populations.
Strong interest in improving healthcare access, affordability, and patient experience.
