An established adult primary-care practice in scenic northwestern Connecticut is seeking an experienced Nurse Practitioner or Physician Assistant to join its collaborative outpatient team.
This is an excellent opportunity for a provider who wants a predictable four-day schedule, greater ownership of patient care, and the resources of a large, fully integrated nonprofit healthcare system. The selected provider will build and independently manage a continuity panel while receiving structured orientation, clinical support, and access to an extensive network of physicians and specialists.
Position Highlights

Position Responsibilities
The Nurse Practitioner or Physician Assistant will provide comprehensive, patient-centered primary care to an established and growing adult population.
Responsibilities include:
- Conducting annual physical examinations and preventive-health visits
- Evaluating and treating acute and chronic medical conditions
- Developing individualized medical, educational, and psychosocial care plans
- Ordering and interpreting laboratory studies, imaging, and diagnostic testing
- Prescribing and managing medications and treatment plans
- Coordinating referrals and collaborating with physicians and specialists
- Educating patients regarding diagnoses, treatment options, preventive care, and lifestyle changes
- Identifying conditions outside the provider’s scope and referring appropriately
- Maintaining accurate and timely documentation in Epic
- Independently managing a continuity panel
- Maintaining an average volume of at least 18 patient visits per day

Qualifications
- Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant
- Appropriate national certification or board eligibility
- Current or eligible unrestricted Connecticut APRN or Physician Assistant license
- Minimum of two years of experience in traditional primary care or a comparable panel-based outpatient setting
- Demonstrated experience independently managing a patient panel
- Ability to maintain an average volume of at least 18 patient visits per day
- Strong clinical judgment and communication skills
