Nurse Practitioner – Post-Acute Care
We are seeking a compassionate and clinically confident Nurse Practitioner to join a collaborative healthcare team providing high-quality, patient-centered care within post-acute care facilities.
This full-time, Monday–Friday opportunity offers a consistent weekday schedule, strong clinical support, competitive compensation, and the ability to make a meaningful impact on medically complex patients through proactive care management and coordinated treatment planning.
Schedule
- Full-time, Monday–Friday
- Consistent weekday/day-shift schedule
- Approximately six hours per day providing care within assigned facilities
- Remaining time dedicated to documentation, care coordination, and related administrative responsibilities
Key Responsibilities
- Perform patient rounds within post-acute care facilities.
- Complete comprehensive assessments, histories, and physical examinations.
- Diagnose and manage acute and chronic medical conditions.
- Develop and update individualized treatment plans.
- Monitor changes in patient status and coordinate appropriate interventions.
- Order and interpret laboratory testing, imaging, and other diagnostic studies.
- Prescribe medications and treatments within the provider’s scope of practice.
- Collaborate with physicians, nursing teams, Directors of Nursing, therapists, and facility leadership.
- Participate in interdisciplinary care planning and clinical discussions.
- Maintain timely, accurate, and compliant documentation within the EMR.
- Support quality improvement, continuity of care, and reduced hospital readmissions.
Required Qualifications
- Active Nurse Practitioner license in the applicable state.
- Current board certification as a Nurse Practitioner.
- Active DEA registration or ability to obtain one.
- Strong clinical assessment and decision-making abilities.
- Excellent communication, organization, and collaboration skills.
- Ability to work independently while functioning effectively within an interdisciplinary team.
Preferred Experience
- One or more years of relevant clinical experience preferred.
- New graduate Nurse Practitioners are encouraged to apply.
- Experience in one or more of the following areas is beneficial:
- Post-acute care
- Geriatrics
- Internal medicine
- Hospital medicine
- Emergency medicine
- ICU or critical care
- Primary care
- Skilled nursing or rehabilitation
Compensation and Benefits
- $120,000–$155,000 annually, depending on experience
