We are seeking a dedicated and skilled Nurse Practitioner to join our team at WCS Healthcare Partners. This role involves providing comprehensive care to residents across two skilled nursing communities, focusing on subacute, long-term care, assisted living (including memory care), and independent living environments. The ideal candidate will have a strong background in managing complex cases and exhibit excellent communication skills in a family-facing environment.
Responsibilities
- Conduct morning rounds focusing on subacute residents and those with priority needs.
- Hold afternoon office hours dedicated to independent living patients.
- Collaborate with telehealth partners to ensure seamless after-hours coverage.
- Provide occasional coverage when the attending physician is on vacation, with compensation.
Requirements
- Certified as an FNP, ANP, or AGNP.
- At least 1 year of experience in subacute or hospital settings dealing with complex cases.
- Ability to flex across different levels of care.
- Strong communication skills in a family-facing environment.
