Family Nurse Practitioner Opportunity – San Antonio, Texas

The Family Nurse Practitioner will provide comprehensive primary care services, including evaluation, diagnosis, treatment, preventive care, and chronic disease management for patients across the lifespan.

Job Responsibilities
Provide comprehensive primary care services to patients of all ages
Evaluate, diagnose, and treat acute and chronic medical conditions
Develop individualized treatment plans based on patient needs
Provide preventive care services, wellness education, and health screenings
Manage chronic conditions including diabetes, hypertension, and other common illnesses
Educate patients and families regarding health management and lifestyle improvements
Collaborate with physicians, specialists, and multidisciplinary healthcare teams
Maintain accurate and timely documentation within the electronic medical record (EMR)
Participate in quality improvement initiatives and clinical practice development
Qualifications
Active or pending Texas APRN license
Board Certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
Master’s or Doctorate degree in Nursing
Strong clinical judgment and patient-care skills
Excellent communication and interpersonal abilities
Commitment to teamwork and delivering exceptional patient care
