We are seeking a Family Medicine Nurse Practitioner (FNP) who is passionate about providing high-quality, compassionate care to underserved populations. This is an excellent opportunity for both experienced providers and new graduates looking to make a meaningful impact in their community.

Responsibilities:

• Provide comprehensive primary care services to patients across the lifespan.
• Conduct physical examinations, assessments, and diagnostic evaluations.
• Diagnose and treat acute and chronic medical conditions.
• Develop and implement individualized treatment plans.
• Prescribe medications and therapies in accordance with state regulations and organizational policies.
• Educate patients on preventive care, health maintenance, and disease management.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to ensure continuity of care.
• Maintain accurate and timely medical documentation within the electronic health record (EHR) system.
• Promote a patient-centered approach that supports health equity and access to care.

Qualifications:

• Master’s or Doctorate degree from an accredited Nurse Practitioner program.
• Current Nurse Practitioner license and certification as a Family Nurse Practitioner (FNP) or eligible to obtain.
• Strong commitment to serving underserved and diverse patient populations.
• Excellent communication, interpersonal, and clinical skills.
• Bilingual skills are a plus but not required.
