We are seeking a dedicated and compassionate healthcare professional to join our team in a 100% outpatient role at our health center. The ideal candidate will work with a diverse patient population, providing comprehensive care and collaborating with other healthcare providers to ensure optimal patient outcomes.
Job Function
This position focuses on seeing patients at the health center, with a slow ramp-up during training, eventually leading to managing a full panel of 15-20 scheduled patients per day. Responsibilities include evaluation, diagnosis, medication assessments and adjustments, patient education, and treatment planning.
Working Hours
Monday to Friday, 8 AM to 5 PM

Job Requirements
- CT State license, DEA, CSR (active or applied preferred, but not required)
- Bilingual language skills in Spanish preferred, but not required
