Primary Care Nurse Practitioner (APRN) – Starting at $120K | Flexible Schedule | Loan Repayment

A mission-driven nonprofit community health organization in Northwestern Connecticut is seeking an Advanced Practice Registered Nurse to join its collaborative outpatient care team.

This opportunity is ideal for an APRN who values patient-centered care, interdisciplinary teamwork and the opportunity to make a meaningful difference in an underserved community. The incoming provider will assess, diagnose and treat patients while coordinating care with physicians, nurses, medical assistants, case managers and other clinical professionals.

Position Highlights

  • Starting compensation of $120,000 annually
  • Flexible scheduling designed to support work-life balance
  • Collaborative, team-based outpatient practice
  • Comprehensive medical, dental and vision insurance
  • Generous vacation, sick and personal time
  • Nine paid holidays
  • 403(b) retirement plan with company match
  • Disability and life insurance options
  • Loan repayment and forgiveness program eligibility
  • Career advancement and professional-development opportunities

Responsibilities

  • Provide direct clinical care within the scope of the Nurse Practitioner license
  • Obtain and document complete patient medical histories
  • Conduct physical examinations and clinical assessments
  • Order laboratory and diagnostic testing and evaluate results
  • Diagnose illnesses and other healthcare needs
  • Develop and support individualized patient-care plans
  • Prescribe and manage appropriate treatment within the provider’s authorized scope
  • Coordinate referrals for specialty services and additional diagnostic care
  • Collaborate with physicians, nurses, medical assistants, case managers and patient-access representatives
  • Help ensure efficient, high-quality and coordinated patient-care delivery
  • Participate in quality-improvement, professional-practice and staff meetings
  • Support community health initiatives, outreach activities and health fairs as needed
  • Maintain current clinical knowledge through continuing education and professional development

Qualifications

  • Graduate degree in nursing or a related field recognized for APRN certification
  • Current or eligible Connecticut APRN license
  • Current Connecticut Registered Nurse license
  • Current qualifying national Nurse Practitioner certification
  • Strong clinical assessment, diagnostic and patient-care skills
  • Ability to work effectively within a multidisciplinary clinical team
  • Commitment to providing compassionate care to diverse and underserved patient populations

Connecticut APRNs who have not completed the state’s experience threshold must practice under an appropriate physician-collaboration arrangement. The organization’s disclosure specifically addresses this collaborative practice structure. Connecticut currently permits independent practice after an APRN has met the applicable three-year and 2,000-hour requirements and completed the required notice process.

Practice Environment

The APRN will work in an integrated outpatient community health setting focused on prevention, early intervention and whole-person care. Providers have access to an interdisciplinary network of clinical and supportive services, creating opportunities to coordinate care across multiple patient needs.

Compensation and Benefits

Starting compensation begins at $120,000 annually.

Benefits include generous paid time off, nine paid holidays, a 403(b) retirement plan with company match, medical, dental and vision insurance, disability and life insurance options, flexible scheduling, professional-development opportunities and potential eligibility for loan repayment or forgiveness programs.
