The Opportunity:

This isn't your typical SNF role. You'll be at the forefront of our acute intervention program, working with clinical autonomy and physician support to manage complex patients right where they live. By combining in-person rounds with strategic telehealth coverage, you'll stabilize residents during critical moments and prevent the trauma and disruption of unnecessary hospital transfers.

Job Responsibilities

What your day look like:
• Lead daily in-person rounds at skilled nursing facilities, building relationships with residents and staff
• Respond to acute changes in condition with confidence and clinical expertise
• Perform urgent assessments and make critical care decisions in real-time
• Manage IV therapy protocols including fluids, Lasix, antibiotics, steroids, and antiemetics
• Provide limited telehealth support to extend your impact across our regional network
• Drive measurable outcomes by keeping residents safe, stable, and home

Qualifications

Requirements:
• Active Ohio Nurse Practitioner license
• Strong clinical assessment skills
