• Seeking an experienced Nurse Practitioner to provide primary care services to residents in assisted living facilities across the Albany region.
• Opportunity available on a part-time or full-time basis with flexible scheduling.
• Ideal for providers passionate about geriatric medicine and building lasting patient relationships.
• Work independently while collaborating with facility staff and healthcare professionals.
Job Responsibilities
• Diagnose and manage acute and chronic health conditions.
• Develop individualized care plans and treatment strategies.
• Prescribe medications and monitor patient outcomes.
• Coordinate care with specialists, facility staff, and families.
• Provide preventive care and health education.
• Maintain accurate and timely medical documentation.
• Ensure compliance with regulatory and clinical standards.
Qualifications
• Active New York State NP License.
• Active DEA Registration.
• Currently enrolled and credentialed with Medicare and Medicaid (required).
• Minimum 3,600 clinical practice hours to practice independently in New York State.
• Primary Care experience required.
• Geriatric or Assisted Living experience preferred.
Working Hours
• Full-Time: 4–5 full days per week.
• Flexible scheduling available.
• Expected start within 4–6 weeks following credentialing and onboarding.
