Nurse Practitioner (FNP) – Primary Care & Behavioral Health | No weekends or on-call!

Salary Ranges between: $125k-$160k (varies depending on years of experience)

We’re seeking a compassionate and motivated Nurse Practitioner (FNP) to provide high-quality care in an outpatient community health setting.

What You’ll Be Doing:

• Conduct comprehensive patient evaluations, including physical exams and health assessments
• Collaborate with physicians and specialists to ensure coordinated, well-rounded care
• Create and manage individualized treatment plans tailored to patient needs
• Prescribe medications and monitor treatment effectiveness
• Support patients with both primary care and behavioral health concerns
• Coordinate referrals to specialty providers and external services
• Contribute to the development and improvement of clinical care processes

Requirements:

• Active New York State license as a Family Nurse Practitioner (FNP)
• Master’s degree in Nursing
• Minimum of 3 years of experience in a primary care or outpatient setting
• Bilingual English/Spanish is a plus
