Salary range: $145,000 – $180,000 depending on experience
A growing community-based healthcare organization in Brooklyn is seeking an experienced Nurse Practitioner to join its clinical team. This position will provide comprehensive primary care services to a diverse patient population while supporting preventive health and chronic disease management.
What You’ll Do
- Conduct patient evaluations, medical histories, and physical examinations
- Diagnose and manage acute and chronic medical conditions, including diabetes and other long-term illnesses
- Develop individualized treatment plans and medication management strategies
- Order and interpret diagnostic tests and laboratory results
- Provide patient education on disease prevention, treatment plans, and lifestyle management
- Coordinate care by referring patients to specialists when appropriate
- Collaborate closely with physicians, nurses, and clinical staff to ensure high-quality patient care
- Document patient encounters accurately in the electronic medical record
- Follow up with patients to monitor progress and adjust treatment plans as needed
- Participate in clinical meetings and quality improvement initiatives
- Maintain patient confidentiality and comply with healthcare regulations
Qualifications
- Graduate of an accredited Nurse Practitioner program
- Active New York State NP license
- Minimum 3 years of clinical experience as a Nurse Practitioner
- Experience managing chronic diseases such as diabetes, hypertension, and other long-term conditions
- Comfortable working with diverse and underserved patient populations
- Strong communication and patient education skills
- Must speak Spanish
Schedule & Work Environment
- Full-time or part-time opportunities available
- Flexible schedule with occasional weekend coverage (approximately once per month)
