Position Summary
The facility is seeking a Family Practice Nurse Practitioner to join its growing primary care team in Bronx, New York. This is a full-time opportunity .
This is an excellent opportunity to practice in a collaborative, multi-specialty environment with strong referral support and an established patient base. The Nurse Practitioner will provide comprehensive primary care services to patients across the lifespan, focusing on preventive care, chronic disease management, and acute illness treatment within a patient-centered, evidence-based model.

Essential Position Responsibilities
Provide comprehensive outpatient primary care services to pediatric, adult, and geriatric patients.
Conduct physical examinations, health assessments, and preventive screenings.
Diagnose and manage acute and chronic medical conditions such as hypertension, diabetes, asthma, hyperlipidemia, and other common primary care conditions.
Order, perform, and interpret diagnostic tests and laboratory studies.
Prescribe medications and develop individualized treatment plans in accordance with New York state regulations.
Provide patient education on disease prevention, wellness, and lifestyle modifications.
Collaborate with physicians, specialists, and interdisciplinary team members to ensure coordinated, high-quality care.
Maintain accurate and timely documentation within the electronic health record (EHR).
Participate in quality improvement initiatives and team meetings.
Qualifications
Education:
Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program.
Certification:
Board Certified as a Family Nurse Practitioner (FNP).
Licensure:
Active or eligible Nurse Practitioner license in New York.
DEA registration (or eligibility).
Experience:
Prior experience in family practice or primary care preferred.
New graduates with strong clinical training are encouraged to apply.
