Annual Salary: $145k – $170k (varies depending on years of experience)

We are seeking a Primary Care Nurse Practitioner to join a dynamic and mission-driven primary care practice located in Manhattan.

Key Responsibilities

  • Deliver high-quality primary and preventive care to a culturally diverse adult and family population.
  • Conduct comprehensive assessments, manage acute and chronic conditions, and support health promotion and disease prevention initiatives.
  • Collaborate closely with physicians, nurses, and allied health professionals to ensure integrated, team-based care.
  • Engage in care coordination, population health initiatives, and quality improvement projects that support clinical excellence and sustainability.
  • Participate in the development and implementation of clinical policies and best practices within the primary care team.
  • Contribute to the center’s patient-centered medical home model, emphasizing access, communication, and continuity of care.
  • Participate in peer reviews, staff training, and committee meetings to advance patient care initiatives and quality metrics.
  • Provide culturally sensitive care, ensuring communication and treatment approaches are accessible to Spanish-speaking patients.

Qualifications

  • Master’s degree or higher from an accredited Nurse Practitioner or Family Nurse Practitioner program.
  • Active New York State NP license and board certification in Family, Adult, or Primary Care (FNP, ANP, AGNP, or ENP).
  • Fluency in Spanish and English (medical or conversational proficiency required).
  • Prior experience in family medicine, internal medicine, or community-based primary care preferred.
  • Strong communication, teamwork, and organizational skills with a commitment to equitable, patient-centered care.
