Family or Adult-Gerontology Nurse Practitioner
📍 Grafton, OH 44044
🕒 Full-Time | 40 Hours/Week
📆 4+ Months Contract – Long-Term Opportunity
💲 $70–$85/hour
We are seeking a Family or Adult-Gerontology Nurse Practitioner to join a collaborative healthcare team in Grafton, Ohio. This is a weekday-only position with no evenings, weekends, or holidays, offering excellent work–life balance and schedule flexibility based on facility needs.
Position Highlights
• Consistent Monday–Friday schedule
• Daytime hours only
• Competitive hourly pay

• Supportive medical leadership and structured clinical environment
Key Responsibilities
• Manage and monitor chronic illnesses, intervening as needed in accordance with ODRC policies and clinical protocols
• Perform routine physical exams, assess chronic condition control, and diagnose new medical conditions
• Prescribe medications, administer treatments, provide immunizations, and perform minor procedures as permitted by training and competency
• Deliver both routine and emergency medical care within scope of practice
• Order, review, and interpret laboratory tests, imaging, and other diagnostic studies
• Evaluate the need for specialty referrals and review consult reports for approval or follow-up
• Conduct infirmary rounds as directed by the Chief Medical Officer (CMO)
• Collaborate with the CMO on medical reclassification requests and non-formulary medication approvals
• Accurately document patient assessments, diagnoses, treatment plans, and follow-up in compliance with ODRC and American Correctional Association standards
• Participate in quality oversight initiatives and work collaboratively with medical leadership
Required Qualifications
• Active Ohio Nurse Practitioner license
• Board certification (FNP or AGNP)
• DEA registration
• BLS/CPR certification (healthcare provider level)
• Comprehensive resume including:
o Education (institution, location, and graduation year)
o Training
o Complete work history
