Primary Care Nurse Practitioner / Physician Assistant
Position Summary
The Primary Care Nurse Practitioner or Physician Assistant provides comprehensive, patient-centered primary care services to individuals across the lifespan in an outpatient setting. Working collaboratively with physicians and the healthcare team, the NP/PA delivers preventive care, diagnoses and manages acute and chronic conditions, and promotes long-term health and wellness.
Key Responsibilities

Perform comprehensive patient evaluations, including histories and physical examinations

Diagnose and manage acute and chronic medical conditions

Provide preventive care, health screenings, and wellness visits

Develop and implement individualized treatment plans

Prescribe medications and manage ongoing therapies in accordance with clinical guidelines

Order, interpret, and document diagnostic tests, laboratory studies, and imaging

Provide patient education on disease management, lifestyle modification, and preventive health

Coordinate care with specialists, community resources, and ancillary services

Manage follow-up care and chronic disease monitoring

Ensure accurate, timely documentation in the electronic medical record

Participate in quality improvement, population health, and patient safety initiatives
Patient Population
-
Pediatric, adult, and geriatric patients (scope may vary by practice)
-
Conditions may include hypertension, diabetes, hyperlipidemia, asthma, depression, acute infections, and preventive health needs
Schedule
-
Day shift, outpatient clinic hours
-
May include occasional evenings or weekends depending on practice needs
Qualifications
-
Graduate of an accredited Nurse Practitioner or Physician Assistant program

Current national certification as an NP or PA
-
Active or eligible state licensure
-
BLS certification required
-
Prior primary care, family medicine, or internal medicine experience preferred

Ability to manage a diverse patient population and multiple conditions
