You’ll step into an outpatient, panel-based primary care role focused on caring for patients with complex medical needs in collaboration with an integrated care team. You’ll work alongside dedicated medical assistants, RN support, behavioral health, chiropractic services, and telehealth consult resources to deliver comprehensive, community-centered care.

Compensation & Benefits
• Base salary ranging from $148,627.20 to $168,553.60 annually
• $12,400 annual access bonus for panel target achievement
• Up to $44,000 additional annual production bonus potential
• $25,000 sign-on plus retention bonuses for partnership-eligible providers
• Alternative sign-on and retention structure for non-partnership track
• $10,000 relocation reimbursement
