Position Overview
We are seeking a Nurse Practitioner (NP) to join a small, patient-centered, non-corporate primary care practice with additional clinical responsibilities at a nearby skilled nursing facility. This part-time (20 hours/week) role is ideal for an experienced provider seeking a consistent schedule, meaningful patient relationships, and a balanced pace of care.
Primary Care Responsibilities

Adult-focused primary care (minimal pediatrics; occasional patients ages 14–18)

Management of chronic conditions, preventive care, and follow-up visits

Collaborative, relationship-based care in a small practice setting
Patient Volume:

16–20 patients per full day

Long-term expectation of ~4 patients per hour, increasing with experience
Skilled Nursing Facility Responsibilities

Provide medical care to skilled nursing and long-term care patients

Patient population includes:
-
Recently discharged hospital patients
-
Medically complex cases
-
Patients undergoing rehabilitation services

Documentation completed in PointClickCare
Patient Volume:

Approximately 8–10 patients per half day
-
Slower pace than the office setting due to patient complexity
Flexibility:

The 4-hour nursing facility block may be scheduled at a time that best fits the provider’s workflow

Facility is approximately 15 minutes from the primary care office
Schedule
Part-Time: 20 Hours per Week (2½ Days)

Tuesday: 9:00 AM – 5:00 PM (½ day office, ½ day nursing facility)

Wednesday: 9:00 AM – 5:00 PM (½ day office, ½ day nursing facility)

Thursday: 9:00 AM – 1:00 PM (½ day office)

Potential for one Saturday per month (9:00 AM – 12:00 PM)
Required Qualifications

Minimum 1 year of experience as an Advanced Practice Provider

Active New Jersey Nurse Practitioner license

Active CDS and DEA
