Nurse Practitioner - Kittanning, PA

Position Overview

We are seeking a dedicated Nurse Practitioner to provide high-quality primary and preventative care to patients residing in a Rehabilitation and Nursing Center. This role offers the opportunity to work closely with interdisciplinary teams and make a meaningful impact on patients with complex and chronic medical needs.

Job Responsibilities

Provide primary and preventative care for long-term and short term care residents
Manage residents experiencing acute changes in condition
Perform advanced care planning and chronic care management
Prescribe medications, including controlled substances, in compliance with Pennsylvania law
Order, review, and follow up on diagnostic testing and treatments
Communicate regularly with nursing home staff regarding patient status and care plans
Collaborate closely with primary care physicians and interdisciplinary care teams
Document care and manage patient records using an EMR system

Qualifications

Current, unrestricted Nurse Practitioner license in the state of Pennsylvania
Board Certification
Active DEA licensure
Clinical background in Adult, Family, or Geriatric specialties
Prior experience in a long-term care or skilled nursing facility setting
Proficiency with electronic medical records (EMR)
Ability to manage patients with advanced illness and multiple chronic conditions

Working Hours

Schedule
Monday–Friday, 8:00 AM – 5:00 PM
Full-time (40 hours per week, 5 days/week)
No on-call requirements
