A facility-based, interdisciplinary medical team is seeking a Nurse Practitioner to provide high-quality primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively with nursing staff and physicians.
Job Responsibilities
Manage residents experiencing acute changes in condition
Support advanced care planning and chronic care management
Prescribe medications, including controlled substances, in accordance with Ohio law
Order, review, and follow up on diagnostic testing and treatments
Communicate regularly with nursing facility staff regarding changes in condition and care plans
Collaborate closely with primary care physicians and the interdisciplinary care team
Qualifications
Active DEA license
Clinical background in adult, family, or geriatric care
Prior experience in a long-term care or skilled nursing setting
Ability to manage patients with advanced illness and multiple chronic conditions
Strong collaboration skills within an interdisciplinary care team
