Position Summary
The organization is seeking a dedicated and compassionate Nurse Practitioner (NP) to join its full-time nursing home and house call team in Manhattan, NY .
This is a full-time, outpatient-focused role providing medical care to patients in skilled nursing facilities and home-based settings . The Nurse Practitioner will manage acute and chronic conditions for elderly and medically complex patients, with a strong focus on continuity of care, preventive services, and reducing unnecessary hospitalizations.

Essential Position Responsibilities
Provide comprehensive medical care to patients in nursing homes and home-based settings
Perform history and physical examinations, routine follow-ups, and urgent evaluations
Diagnose and manage acute and chronic medical conditions
Order, interpret, and evaluate diagnostic tests and labs
Develop and manage individualized treatment and care plans
Coordinate care with primary care physicians, specialists, nursing staff, and caregivers
Educate patients and families on disease management, medications, and preventive care
Maintain accurate and timely documentation in the electronic health record (EHR)
Participate in care coordination, quality improvement initiatives, and population health efforts
Qualifications
Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP)
Licensure & Certification: Active or eligible New York State Nurse Practitioner license; national certification required (FNP, AGNP, or GNP preferred)
Experience: Prior experience in nursing home, house call, geriatrics, or primary care preferred; new graduates are welcome to apply
