Family or Adult-Gerontology Nurse Practitioner
Location: Grafton, OH
Shift: Weekdays (flexible start/end times; no evenings, weekends, or holidays)
Duration: Approximately 4 months (may extend)
Hours/Week: 40
Position Overview
Seeking a Family or Adult-Gerontology Nurse Practitioner licensed in Ohio with at least 1 year of NP experience in a primary care or hospital medicine setting. The NP will provide on-site primary care services to the male population in a correctional facility.
Key Responsibilities
• Monitor and intervene in the course of chronic illnesses using facility policies and protocols.
• Conduct routine physical exams and diagnose/manage acute and chronic conditions.
• Inoculate or vaccinate against communicable diseases; prescribe medications and perform minor procedures within scope of practice.
• Evaluate the need for specialty referrals and review consult reports for approval.
• Order and review laboratory, x-ray, and other diagnostic tests.
• Collaborate with medical leadership on medical reclassifications and non-formulary medication requests.
• Provide routine and emergency medical care as scheduled.
• Conduct rounds on infirmary patients as appropriate.
• Document all assessments, diagnostic impressions, treatment plans, and follow-up in accordance with healthcare and correctional standards.
• Cooperate with medical leadership for quality oversight and follow guidance.
Required Skills & Experience
• Active Ohio NP license.
• Board certification (ANCC preferred).
• DEA certification as required for prescribing.
• At least 1 year of NP experience in primary care or hospital medicine.
• Strong knowledge of patient assessment, diagnosis, treatment planning
