This position reports to the Market Leader and Clinical Manager with accountability for providing strategy, judgment, organization, and evidence-based analysis to influence decisions, and directly to meet Care at Home’s requirements. Job Responsibilities
The Nurse Practitioner will have the following responsibilities:
In partnership with the clinical team, provide medical highest quality medical care for the practice’s patients.
In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).
Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.
Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.
Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.
Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).
Provide back up call to an overnight, holiday and weekend nurse line for clinical escalations in multiple states.
Licensing requirement:
Willingness to obtain and maintain Nurse Practitioner licensure in multiple states to support on-call backup coverage, with all associated licensing costs covered by the company.
Perform other job-related duties as assigned.
Qualifications
Bachelor’s or equivalent degree with a record of strong academic achievement.
Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
Certification & Licensure: Board certified (American Association of Nurse Practitioners [AANP] or American Nurses Credentialing Center [ANCC]) and holds active state licensure in the state of practice.
Prescriptive authority/authorization in the State of New York including the ability to obtain DEA controlled substance registration.
Valid NPI and American Heart Association BLS Certification.
Experience: At least 3 years of active practice required. DEA Certification required.
Communication:
Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner.
Bilingual, English & Spanish Preferred.
Relationships: Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents.
Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.
